Do I Need a Weight Bearing MRI?

A Weight Bearing MRI may be recommended when standard MRI results do not fully explain a patient's symptoms — particularly back or neck pain that is significantly worse in an upright position. Here is a guide to help you understand whether this specialized scan might be appropriate for you.

Signs a Weight Bearing MRI May Be Needed

• You have back, neck, or leg pain that is notably worse when standing, walking, or sitting for extended periods
• Your symptoms are significantly relieved when you lie down
• Your standard MRI appears normal or only mildly abnormal despite significant symptoms
• Your physician suspects positional spinal stenosis or dynamic disc herniation
• You are being evaluated for spondylolisthesis or spinal instability
• Post-surgical assessment is needed to evaluate spinal fusion or decompression under functional load

Conditions Best Evaluated with Weight Bearing MRI

• Lumbar spinal stenosis: Narrowing of the spinal canal that may worsen under axial compression
• Disc herniations: Discs that bulge or herniate more prominently under load
• Spondylolisthesis: Vertebral slippage that may change in severity with posture
• Degenerative disc disease: Disc height loss and segmental instability that is more apparent under weight-bearing conditions

When Standard MRI Is Sufficient

Many conditions — including brain lesions, joint pathology, and most soft tissue abnormalities — are best evaluated with a standard MRI in the supine position. A Weight Bearing MRI is specifically designed to address functional spinal questions. Your referring physician is the best person to determine which scan is appropriate for your clinical situation.
